5. Muscle Aches, Diarrhea, Fatigue, Headache, Runny Nose, Sore Throat – with a Y – N to circle
with the answer
6. Temperature History – this column will display the two most recent temperatures for the
resident
7. Temperature – for the staff to record the observed temperature
8. Any additional comments.
