In an event where you need to review or make any changes to your resident's medication orders, please refer to the below article which will help you manage medication.
The article will help you carry out the following actions:
- Medication Profile Display
- Filter Medication
- Manage Medication Order
- Modify Medication: Put on Hold, Discontinue Order or Stop Order
- How to Audit changes made to a Medication Order
- Login to your account.
- Go to e-MAR > Orders > Medication Profile
- Search for and select your resident.
- Start by typing in the first name of the resident you need to check for. You can also search for your residents by their admission status.
Medication Profile Display
- The new medication profile will display medication in the following way and will include Order Status and the Medcation Schedule.
Filter Medication
- Order Status: On your resident Medication Profile, next to Order Status, you are able to filter the necessary medication you need to manage in the drop down list. Please note that you can choose more than one order staus at once.
-All
-Active
-Discontinued
-Ended Orders
-Future Orders
-Hold Orders - Order Type: You can filter the medication orders by either MAR or TAR orders
- Only: You can choose to filter your medication orders to view only between the following. Please note that you can choose to display more than one at a time.
-Controlled Drug Orders
-PRN Protocol
-High Alert Orders
-Self-admin Orders
Manage Medication Order
- Update Instruction:
- On the Manage Order Screen, click Edit to update the medication Instructions, Route of Administration, place medication on Self Admin, or to place medication on High Alert.
- Click Save all Edits to update details. - Edit Schedule:
- To expire a medication order, click on End Schedules to automatically apply the current date and time as an end date to the schedules.
- You can also apply a different end date, by selecting this next to Ending
- You can add a Give Time and add PRN to the schedule
- You can change the Give Amount by entering a new amount
- Click Save all Edits to update details. - Edit Stock Amount
- Review and edit the inventory by adjusting the On-Hand, Brought from Prior, Return/Destroyed or Carried To Next. Any changes made directly to the on-hand amount will show underneath next to On Hand Adjustments.
- Click Save all Edits to update details. - Preview
Once you have made all the necessary changes to your medication order, you can preview the new schedule before proceeding to Save All Edits.
Modify Medication:
Put on Hold, Discontinue Order or Stop Order
Under Modify, you can choose to Put on Hold, Discontinue Order or Stop Order. Please click on the button which is appropriate to the action you want to carry out on the medication order.
Put on Hold
Enter the required details including the dates you wish to apply the modification from and to.
Discontinue Order
Please enter a Discontinued From date and time and click Save all Edits
Stop Order
Under Stop Order, please select a reason before clicking on Save all Edits. Stop order will apply the current date and time and will immediately expire the medication from the Medication Profile.
Confirm Manage Order:
Select a reason and sign off the changes to update this successfully
How to Audit changes made to a Medication Order
In an event where you need to review changes that have been made to the medication order, you can use the Manage Order History screen to do this. If any changes have been made to a medication order, under Manage Order, the Last Updated box will appear on the upper right side of the screen. If no changes have been made, then you will not see this.
- Login to MED ecare
- Go to e-MAR > Orders > Medication Profile
- Search for and select your resident.
Start by typing in the first name of the resident you need to check for. You can also search for your residents by their admission status. - Find the Medication you want to review, then click Manage
- On the Manage Order screen, if changes have been made, then you will see a Last Updated box on the right side of the screen.
- Click View History to review the changes that have been applied to that medication order.
On the Manage Order screen you can choose to filter the reasons for your audit in multiple ways.
- Choose to show history of 'This Order' or 'Entire Chain'
For instance, you may want to filter with the Entire Chain to audit any changes made to the medication stock on a previous medication cycle. - You can select the Type of audit you wish to carry out on the medication order. You can filter out the following option:
-End Schedule
-Instruction
-Medication Alert changes
-Schedule Modifications
-New Schedule's created - You also can review changes made by a particular user or a witness. Please use the drop down menu and select the appropriate user to audit.
Please note only users who have made changes on the order will show up here.
