In an event when you have recorded information in a completed Assessment and want to share this with your resident's Care Plan, you can do this using MED eCare's Relationship Builder feature.
The Relationship Builder uses MED e-Care's 'Push Technology' to transfer data from e-Assessments to e-Plan which helps you generate a best practice Care Plan and ensures consistency across your resident's clinical documentation.
This article will show you:
- How to use the Relationship Builder window to push data from a completed Assessment into your Care Plan
- Add a progress note whilst building these triggers to your resident's Care Plan items.
Once the Relationship Builder has been enabled in your system you are ready to use the Relationship Builder in your Care Plan. Please contact our support desk to assist with this.
Before you start using the Relationship Builder, you will need to first setup trigger-able facility rules which will populate the Care Plan items according to the Assessment answers. How to setup Relationship Builder facility rules will give you all the information you need to be able to do this.
1. After completing the Assessment, please navigate to the resident's Care Plan and open the current Care Plan.
After you have opened the resident's Care Plan, a window will pop-up on your screen. This will show you what has been triggered on the completed Assessment, in this instance, it is showing you rules created for the Brief Falls Risk Assessment.
On the left side of the Relationship Builder, it shows you all the Care Plan items where triggers exist. You can navigate between each are to build these rules to your Care Plan.
2. Building the triggers to your Care Plan items
On the left hand side it will show you triggered Etiologies, Outcomes and Interventions with empty tick boxes next to them. You can select which triggers you want to add your Care Plan and click Build Selected.
Once you have added the relevant triggers, these will be displayed on the right hand side, under Current Care Plan.
If the triggers are NOT required for your Care Plan you can remove these by clicking on Remove Selected. You can also use this tool to remove what's no longer applicable and replace it with the new data coming from the Assessment. This ensures that there's always consistent information between your Assessments and Care Plans.
Simultaneously, you can use this opportunity to add in a Progress Note at the top. Once you have entered your note, as you are building the triggered items into the Care Plan, the note you have entered will create a copy of this in e-Notes according to the Care Plan category. In the above case, it would create a note under Falls / Balance category.
