If your care plan is not defaulted to the correct type here are some simple steps you can do in order to fix it.
1. Go to Settings > e-Plan > Options
2. Change the Default Care Plan type to the correct one (in this case it is MED e-care Care Plan)
3. If you would like the creation and management of multiple Care Plan types make sure the highlighted box is checked off. If it is not checked then the system will always default to the type you have selected as your default.
