Your pharmacy process may differ on how they send Warfarin and variable give dose amounts, however, this guide will take you through some of the differences and which process is recommended to follow.

Medication administration determined by a test result

For example Warfarin, Insulin

This will be sent as a zero (0) give dose at the scheduled time of administration. This is to allow the care team to check the yellow anticoagulant book for the latest INR results.

•  You can see the zero (0) give dose amount in the 'amount' box, below right

1. Once all checks have been completed, pop and dot the medication that is required. Do not enter the amount now, wait until the medication has been administered.

2. For Warfarin strengths not required for this administration, click 'not administered' and select 'medication strength not required'. See below Warfarin 3mg

3. Once all medications have been witnessed as administered, click

4. A message will pop up

5. Click on the green administered box to the right of the medication administered and enter the quantity of tablets administered

6. Click save

Variable give doses

A variable give dose is when the instruction from the prescriber differs on the amount to be administered. e.g. Take ONE to TWO tablets, Take 5ml - 10ml, Take 1-2 sachets.

Your pharmacy provider may enter these as zero (0) like the Warfarin above, in which case follow the steps 1, 3 , 4, 5 & 6 as above, or the pharmacy may enter the minimum amount required.

If the minimum amount is entered by the pharmacy, you can see this under the medication name, below left and in the 'amount' box, below right. Please take care to enter the actual amount given. Caution must be taken when using pop and dot as failure to document the correct amount administered will have an adverse effect on stock quantities and audits.

1. Once all medications have been witnessed as administered, click on the green administration for variable dose medication and manually enter the quantity of medication administered

2. Now click  for all other regular medications
